PRIOR TO YOUR FIRST APPOINTMENT
Please arrive at least 15 minutes prior to your appointment in order to complete new patient intake forms. We ask that you allow a minimum of 30 minutes for your initial office visit. Please bring the following items with you to your appointment:
Current health insurance cards
Drivers license or photo ID
List of current medications
Including prescription, non-prescription and over-the-counter
Co-payment
We are required to collect co-payment at the time of service
Upon your arrival, our office will enter or update your contact and insurance information. To avoid delays in registering, please contact your health insurance company at least one week before your appointment and ask:
If you are in network with our provider: Dr. Aamir Mahmood, DPM
If you need a referral from your primary care physician.
If a referral is needed, you will need to ask your primary care physician to have it faxed or mailed to our office ahead of time.
Please ask them to explain your benefits for a specialist office visit and learn if you have a deductible or not.
